Background

In order to participate in the Family Coordinated Entry System (FCES) all households will be required to complete minimal documentation, including the CES Authorization Form, homeless verification forms, including self-certification, Third Party Verification of Homelessness, and the Disabling Condition Form if applicable, which will be uploaded into their HMIS profile.

 

Documentation

CES Authorization Form: Required document to participate in FCES. The Head of Household (HoH) should sign the document either electronically or via wet signature. If verbal consent is given the form must be obtained and uploaded into the HMIS profile within 30 calendar days. 

 

Third-Party Homelessness Verification Form: The minimum document required for a family to be matched to a housing opportunity through FCES. This is completed by the Access Point staff to verify a household's literally homeless situation and length of homelessness.  

 

At-Risk of Homelessness Certification: Access Point staff should complete this form to verify the living situation of households who are At-Risk. 

 

Disabling Condition Form: Optional document to be collected from Head of Household to confirm chronic disability verified by a licensed medical professional.

 

Self-Certification of Homelessness: Optional document for Head of Household to verify up to 3 months of homelessness.
